are aged 75 and over
live in a care home for older people
are aged 12 and over and have a weakened immune system

We offer 50% of our appointments pre-bookable which can be booked by the Care Navigation Team throughout the day.
Our phone lines are on from 8:00-18:30 Monday-Friday.

RIGHT CARE FIRST TIME!
Contacting the surgery patients will be care navigated to the appropriate route for treatment, the pathway could mean patients visiting local Pharmacies, seeing a Physiotherapist, Podiatrist or another Medical Practitioner such as a Physician Associate or Advanced Nurse Practitioner.
Patient’s symptoms are assessed and then advised on how best to access help or support using NHS resources and a wide range of medical professionals.
